Ca-HA microspheres of Xelestal/Xelestal+ are too large for macrophage endocytosis and are broken down by lowering the extracellular pH. This breakdown leads to direct effects of calcium ions on fibroblasts, causing chemotaxis toward higher Ca2+ concentrations. High Ca2+ concentrations promote the activation of genes that stimulate collagen and elastin production in fibroblasts, lead to cell proliferation, transformation into myofibroblasts, and activation of myofibroblasts. Xelestal and Xelestal + products are engineered to maximize neocollagenesis and minimize implant migration.
